Submitting a new UV or UVC Application

The below is required for all United Voice (UV)/United Voice Cloud (UVC) deals, please follow the below. All UVC/UV application submissions must include the following documents scanned together and attached to the email accordingly:

  1. Supporting Documents
    1. Copy of ID of Signatory
    2. Bank Confirmation Letter
    3. Company Registration documents

 

  1. Port Documents
    1. Copy of Supplier invoice reflecting the number(s)
    2. Copy of ID of Signatory
    3. Port letter on company letterhead
    4. GNP authorization form on company letterhead

 

  1. Cancellation Documents (TELKOM ONLY)
    1. Copy of Supplier invoice reflecting the number(s)
    2. Copy of ID of Signatory
    3. Cancellation letter on company letterhead

 

  1. Coverage Check (LTE, Microwave, Fibre)

 

  1. UVC Order Sheet

 

  1. UVC Extension Schedule

 

  1. UV/UVC DMR
    1. Include Special billing instructions under “Purchasing Notes”
